Credit Bureau Authorization


I consent to BMO Financial Group and its agents using my Personal Information to obtain, validate or exchange credit information and other financially related information about me at any time during the term of this agreement from any credit reporting agency, credit bureau, other financial services industry databases, my employer(s), my telecommunications services provider, mobile services provider, or any person who has or may have financial dealings with me for the purposes outlined in the Privacy Code. BMO Financial Group may use information I provide to BMO when opening or operating any accounts I have with BMO to confirm or validate such information with third parties. I agree that if I am issued a Card, I am the only person permitted to use it. I have read the Terms and Conditions and the Privacy Disclosure & Consent below and agree to be bound by them.


Terms and Conditions


I, the Applicant, certify the information provided in this application is true and correct and I am providing my express consent that you (Bank of Montreal) establish a BMO Mastercard Account in my name (the "Mastercard Account"). Please issue me the BMO Mastercard card (the "Card") applied for or such other card as you may approve, and send me renewals, substitutes or replacements issued from time to time at your discretion. I request a Personal Identification Number (PIN) to allow me to use the Card in automated teller machines accessible with the Card and to complete purchases at chip-enabled terminals. I request the services available to holders of the Card that may be issued to me and understand that such services may vary or be terminated and that some services or benefits may only be available to me if I enter into separate agreements or obtain separate authorizations.


If I am approved for a BMO AIR MILES Mastercard card, I consent to you providing LoyaltyOne, Co. or its agents, any information reasonably required for the AIR MILES Reward Program (the "Program"), and if no AIR MILES Collector Number is shown on this form, I consent to you providing LoyaltyOne, Co. with my name and address for enrolment in the Program. By enrolling as a Collector in the AIR MILES Reward Program, I agree to be bound by the Terms and Conditions of the AIR MILES Reward Program and I consent to the collection, use and disclosure of my personal information by LoyaltyOne, Co. in accordance with the AIR MILES Privacy Commitment. I acknowledge that some services or benefits, including the AIR MILES®† Reward Program are supplied by firms independent of you and that you are not responsible or liable for anything in connection with those services or benefits.


I acknowledge that some services or benefits, including the Maple virtual care services of on-demand doctor visits and mental health counselling are supplied by firms independent of you and that you are not responsible or liable for anything in connection with those services or benefits. Services and benefits may vary by credit card.


I acknowledge that my name, language preference, and email address, as the owner (and co-owner, as applicable) will be shared with Maple, so that Maple can send me/us an email invitation to create my/our Maple account(s) for access to 5 free virtual care visits (I understand that I can choose to connect with a doctor in minutes or access mental health counselling). I understand that registration is optional. Maple is a Canadian virtual care provider offering access to online doctors, virtual healthcare & prescriptions in Canada. I am providing my consent for you to share this information with Maple if my application for a BMO® World Elite®* Business Mastercard®* is approved. If you issue me a Card, I agree to abide by the Cardholder Agreement, as amended or replaced by you, which you send relating to that Card, and I agree that use of any Card applied for will show I received the Cardholder Agreement.


I will be responsible for all use of, any fees for, or other charges to the BMO Mastercard Account.

Privacy Disclosure & Consent - Your Personal Information

In this Privacy Disclosure & Consent section only, “you/your” shall refer to the Applicant, and “we/us/our” shall refer to BMO Financial Group.


To learn more about how we collect, use, disclose and safeguard your Personal Information, your choices, and the rights you have, please see our Privacy Code (available at bmo.com/privacy, or from any of our branches). BMO also accepts calls made via Telecommunications Relay Service (TRS).


What is Personal Information?


Your Personal Information is information about you that you provided to us or information we collected from other sources such as credit reporting agencies, and includes your name, address, age, financial data, Social Insurance Number, employment information, and other information that could be used to identify you.


Why do we need your Personal Information?


We collect and use your Personal Information to:


  • verify your identity;
  • ensure we have accurate information about you;
  • understand your financial needs (including your eligibility for products and services you requested or accepted or were pre-approved for)
  • to manage our relationship;
  • protect against fraud and manage other risks;
  • communicate with you regarding products and services that may be of interest;
  • understand our customers, including through analytics, and to develop and tailor our products and services;
  • comply with legal or regulatory requirements, or as permitted by law; and
  • respond to questions you may have.

We will also use your Personal Information to make decisions in real time by using tools to automate the processing of your Personal Information, for example, whether to approve credit. These decisions can affect the products, services, or features we may offer you and are also used to protect you from fraud.


If we use your Personal Information for a different purpose, we will identify that purpose.


Sharing your Personal Information


BMO Financial Group consists of Bank of Montreal and its affiliates. Your Personal Information, including information about your authorized representatives and beneficiaries, is shared within BMO Financial Group, to the extent permitted by law, to:


  • ensure we have accurate information about you, and your authorized representatives and beneficiaries;
  • manage our total relationship;
  • provide a better customer experience;
  • meet your needs as they change and grow; and
  • manage our business.

Your Choices


Sharing: You may choose not to allow us to share account-specific information within BMO Financial Group, but you understand we will share your Personal Information where two or more BMO Financial Group affiliates provide you with jointly offered products or services.


Direct marketing: You may choose not to allow us to use your contact information for direct marketing, such as mail, telemarketing or email informing you about products and services we think may be of interest and value to you.
